Lifecare Home Health - - Responsibilities: Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care; Assist the patient with safe transfers and ambulation per Hospice policy; Report changes and document pertinent information and care rendered to patient to ensure continuity of care; Practice infection control principles and maintain a clean, safe environment; Perform on-call/after-hours responsibilities and participate in weekend/holiday coverage rotations
