Lifecare Home Health - - Responsibilities: Create, maintain, and adjust daily and weekly schedules for hospice clinicians and support staff; Coordinate visit coverage across disciplines to support timely and compliant patient care; Respond to scheduling changes, staff callouts, and urgent coverage needs; Monitor scheduled versus completed visits and escalated coverage gaps or risks to the ADPCS; Assist with documentation tracking and scheduling-related reporting as directed including HOPE visit requirements
