Job Description
Job Title: Occupational Therapist (OT)

Hours per Shift: 8 hours (08:00 - 17:00)

Experience: Minimum 1 full year of Adult Home Health experience

License: Must have a valid license in hand

Certifications: CPR, BLS

Must-Have:
- Reliable transportation
- Current driver's license
- Agency-required liability insurance

Description:
Home visits are required for this position.
Periodic office meetings will be held.
Traveling up to 75 miles from home is necessary.
The candidate must be willing to drive to patient homes daily and attend the clinic for meetings.
A vehicle, driver's license, and auto insurance are required.
The average number of patients per day is 5-6.
The assignment includes both Start of Care and Follow-Up cases.
The contractor may be assigned between 5-10 patients per day, equating to roughly 6-8 points per day.
The expectations are high, including accepting all patients scheduled.
Documentation must be completed within 24 hours of each appointment.
It is essential to understand that there are no guaranteed hours with this contract.
Clinicians must meet weekly productivity points of 30, which equates to approximately 40 hours per week.
All overtime must be pre-approved.
If there are not enough patients to meet the financial needs, the contractor can float to another location for additional hours.
Experience with HomeCare HomeBase and OASIS is required.
The candidate must also be proficient in the necessary EMR systems used in home health.
