Lifecare Home Health - - Responsibilities: Provide direct patient care to patient under the direction of the RN; Assist in safe transfers and ambulation; Report changes and document pertinent information to ensure continuity of care; Promote infection control and maintain a clean, safe environment; Participate in on-call/after-hours coverage as assigned
