Lifecare Home Health - - - Responsibilities: Provides direct patient care to patient under the direction of the RN and according to the Aide Plan of Care that supports patient/family needs are met.; Assists the patient with safe transfers and ambulation per the Hospice policy.; Reports changes and documents pertinent information and care rendered to patient to ensure continuity of care.; Practices infection control principles and maintains a clean, safe, and comfortable environment.; Performs on-call/after-hours responsibilities and provides on-call/after-hours services to patients and their families as assigned.
