ResponsibilitiesUnder general supervision, the candidate will perform social services functions at an advanced level to individuals and their families. Assist in engagement, needs identification, care coordination, referrals and linkage, and follow-up with individuals on assigned caseload. Skills in documentation, communication with individuals, family, and community resources expected.
